Action required for disability recipients

Return ‘E&E’ statement to avoid interruption of benefits and health care

Feb. 15, 2024 – OPERS disability benefit recipients are required to report to us any employment during the previous year, as well as total earnings, employer information and job descriptions.

To do so, those recipients are asked to complete the Employment and Earnings Statement, which we began mailing last week. Even if you didn’t work in 2023, you must complete and submit the E&E statement. Only income from employment should be reported; spousal support, child support, and volunteer service are not required information.

However, if you applied for OPERS disability benefits on or after Jan. 7, 2013, you’re required to report any Social Security Disability Income.

If OPERS does not receive your completed statement by April 15, your May disability benefit payment and subsequent payments will be suspended until we receive the form. In addition, as of May 1, your OPERS dental and/or vision coverage (including coverage for any enrolled dependents) will be terminated (if enrolled), and your monthly Health Reimbursement Arrangement deposit will be discontinued (if applicable).

OPERS will waive the statement submission for disability recipients who meet one of the following criteria:

  • Are under legal guardianship
  • Reside in a skilled nursing facility
  • Were age 70 or older as of Dec. 31, 2022

If you misplace the initial Employment and Earnings Statement we are mailing to you, you can log in to your OPERS online account and print a new one. You can return the form to OPERS through the mail or through your online account.

    aFTER i LOG IN TO MY opers ONLINE ACCOUNT, Where do I find the Statement of Employment and Earnings after Receipt of a disability benefit EE-POST form?

    I signed into my account, but couldn’t find the link for Disability Statement of Employment and Earnings to fill out.

      Beginning this year there is not a paper Disability Statement of Employment and Earnings available to print and complete. The Disability Statement of Employment and Earnings is to be submitted electronically through your OPERS online account. Click on the “Complete our Statement of Employment and Earnings After Receipt of a Disability Benefit form now” link located on the Home Page of your online account to submit the online form.

    I have been unable to find this years “Statement of Employment and Earnings After Receipt of a Disability Benefit” form in my mail (if it was sent out). Reading above, I logged into my account, but other than the Welcome, my name, OPERS ID number, etc. at the top center, regardless of what I click on (ie. Account, Home, etc.), it’s a blank (empty) page. Nothing is showing for any of the drop downs, and still no way of knowing where to find the form.

    It’s my understanding that you’re not accepting paper version of the form now. Please advise.

Click on the “Complete Your Statement of Employment and Earnings After Receipt of a Disability Benefit form now” link located on the Home Page of your online account to submit the online form. If you do not have access to complete the online form please call OPERS at 1-800-222-7377 to request that a paper Statement of Employment and Earnings form be mailed to you.
